A Documented System and a Working System Are Not the Same Thing

Your SHMS is impeccable. Your documents and procedures are current, your risk management framework addresses every hazard with stringent controls, and you’re confident the whole system would hold up under audit. That confidence may be entirely justified.

Is it actually working?

Your SHMS says hazards will be reported. Are they?  It says controls will be verified and supervisors will inspect every work area. Is that happening, every shift, on every site?

A permit requiring five managers’ signatures is unlikely to be obtained by your maintenance team on a Sunday. The plant still needs to run. A crew completing a short task at 2am in the rain signs on to a thirty-page risk assessment. Did they read it?

The legislation sets the standard. It doesn’t set the page count. Mineshield’s effectiveness audits find where the gap between the two has become a problem.

Compliance Audit

Checks whether your SHMS exists and meets legislative requirements. Confirms the system is structured correctly and legally sound. Does not test whether it’s applied on site.

Effectiveness Audit

Tests whether your SHMS is actually working in the field. Examines what’s happening on site against what the system requires. Shows where it’s working, where it isn’t, and why.

Field Compliance

Are the procedures and controls of the SHMS being followed in practice?

What We Look At

Hazard reporting, action close-out, control verification, supervisory oversight, fatigue management, PPE, permit processes, competency and training currency.

System Fitness

Is the system actually followable under real working conditions?

The Real Test

Whether the system can actually be followed on site, not just read in an office. If it can't, that's the problem, not the people.

Findings show whether a gap is a compliance issue or a system issue, because the fix for each is different.

What You Receive

A written report which identifies where the system is functioning as intended, where it isn’t, and what’s driving the gap. Findings are categorised and prioritised, giving you a clear basis for action. Where the issue is the system itself rather than how it’s being followed, the report sets out what needs to change.
